About the Role

As a Day Opportunities Facilitator at Cumas you will develop lead and deliver high-quality projects programmes and supports for individuals with intellectual disabilities. Reporting to the Team Leader/Day Opportunities Co-Ordinator you will help participants enjoy a meaningful inclusive life while enhancing their independence in line with their choices and preferences. The role involves building strong professional relationships with individuals families staff and community or statutory agencies and includes mainly daytime hours with occasional evening and weekend work in both in-house and community settings.

Role Requirements

  • Degree in Social Care Social Studies Nursing Youth and Community Studies or Social Work or other relevant qualification at degree level

  • A minimum of 6 months work experience working with individuals with intellectual disability with experience working with individuals with complex needs a distinct advantage.

  • Excellent IT and report writing skills.

  • Must hold a full clean driving licence valid in Ireland and be willing to drive company vehicle

  • Proficient in both oral and written English language

  • Have the legal right to work in Ireland and currently hold valid work authorisation for Ireland

Main Duties and Responsibilities

The Day Service Facilitator will have the following duties:

Provision of Person Centred Supports and Opportunities
  • To facilitate encourage and develop each individuals choice and decision-making skills and ensuring the individuals and their representatives are actively involved in the decision-making process.

  • Develop with assigned Individuals and their Circle of Support their Individual Plan ensuring that the plan is scheduled delivered by those assigned to support them in various aspects of their plan.

  • The Day Service Facilitator will monitor and evaluate the impact of each project/programme/support/opportunity to establish and record how well their experiences impacted on their wellbeing personal growth and development.

  • Accompany and support Individuals on holidays outings and in using community social and recreational facilities as required.

  • Ensure that the will and preference of the Individuals is articulated and informs all supports provided.

  • Provide intimate care supports in a manner which the Individuals privacy dignity choices and preferences are upheld and in line with Cumass intimate care policy and as set out in their Support Plan.

  • To support promote and facilitate meaningful engagement and opportunities to participate a range of valued social roles which enable Individual to contribute their skills and talents.

  • To support promote and enable the Individual to direct their lives enabling them to maximise their independence and control.

  • To ensure the best possible health and wellbeing of individuals in all areas of their lives including healthy eating and promoting a healthy active lifestyle

  • To be an advocate or act as a liaison person for the individuals as instructed by the Team Leader/Day Services Co-ordinator.

  • Ensuring that your work and interaction with others is such that it promotes and enhances the independence of the service users.

  • To support the Team Leader/Day Services Coordinator to identify and respond to unmet needs.

  • To be responsible for the co-ordination preparation and planning of circle of support meeting and annual reviews with individuals where key working responsibilities have been assigned.

  • Respect professional boundaries and maintain confidentiality for all individual and service information.

Project/Programme/Individual Opportunity Planning and Implementation

  • To develop implement and support the evaluation of a range of capacity building projects and programmes both in house and community based which support each service user to increase their social participation and independence at the time of their choosing.

  • Design deliver and evaluate projects and programmes both in house and community based cost effectively and to a high standard of quality.

  • Ensure that projects programmes and opportunities meet then choices preferences and requirements of individuals providing them with meaningful engagement opportunities to engage in their community.

  • Lead/ facilitate projects/programmes and supports in a manner which is adapted to individuals.

  • Ensure team members are aware of the projects/programmes/individual supports required and scheduled for each day and overseeing service delivery.

  • Enabling other staff to deliver maintain and progress scheduled projects and programmes.

  • Ensure service locations are scheduled set up programmes delivered on time; equipment is available on each service day.

  • Ensure service locations are cleaned and left appropriately after use.

  • Update and maintain communication with staff and individuals on the range of available projects and programmes using a variety of traditional and alternative communication methods.

  • Monitor the progress of projects and programmes and provide the guidance to other group staff as needed to ensure successful outcomes.

  • Completing monthly & quarterly feedback/planning and review sessions with Individuals and groups

  • Preparing evaluation reports inclusive of completed programme design templates for approval by Team Leader/Day Services Co-ordinator.

  • Evaluate the impact of each project/programme with a particular focus on how well the programmes impacted on the individuals personal development.

  • Share information with Individuals/staff/volunteers as required by the Team Leader/Day Service Co-ordinator

Maintaining Safe and Effective Work Systems/Record Keeping and Reporting

  • Evidence the planned supports for meaningful engagement in activities through key working sessions daily logs and other service information systems.

  • Be familiar with and ensure that Cumas policies procedures and codes of practice are adhered to.

  • Report and record all complaints concerns incidents accident or near misses in line with the standards.

  • Be responsible for the safe use and care of equipment and report faulty equipment etc. as it arises.

  • Be familiar and operate to fire regulations and emergency procedures and participate in the implementation of same.

  • Actively ensure that standards and practices regarding health and safety are fully adhered to in all areas of Cumas and participate in incident reviews or other investigations as required.

  • Reporting any challenges issues or concerns to Team Leader/Day Services Coordinator

  • Record and report any hazards or accidents to the Team Leader/Day Services Coordinator.

  • Keep relevant and up to date Cumas record systems in line with required standards and within GDPR requirements.

  • Attend and participate fully in regular staff meetings where all aspects of the service area are discussed with a view to maintaining high standards.

  • Attend mandatory and specific skills training courses as scheduled.

  • Demonstrate a commitment to using the resources available to the service in a planned and efficient manner.

  • Provide cover as required.

  • To perform such other duties as may be assigned from time to time by the Team Leader/Day Service Co-ordinator
