Job Summary
The Hospice Admission Coordinator is responsible for coordinating the intake and admission process for new hospice patients ensuring all necessary assessments documentation and legal requirements are completed in a timely and accurate manner. The coordinator works collaboratively with the admissions team physicians social workers and chaplains to ensure a smooth and compassionate transition into hospice care.
Qualifications
- At least 2 years of hospice experience
- Experience in patient admissions case management or healthcare coordination is a plus.
What Youll Do
Patient Admissions:
- Coordinate and manage the intake process for patients being referred for hospice care.
- Conduct initial assessments (clinical physical and psychosocial) for patients to determine eligibility for hospice care.
- Ensure the completion of all necessary forms and documents including consent forms and advanced directives.
Patient and Family Communication:
- Communicate effectively with patients and their families about the hospice program explaining the services benefits and care options available.
- Provide emotional support and educate patients and families about the hospice process what to expect and how to prepare for end-of-life care.
Documentation and Compliance:
- Ensure all required documentation for patient admission is complete and accurate including clinical assessments physician orders and insurance documentation.
- Maintain up-to-date knowledge of state and federal regulations regarding hospice care and ensure compliance with those regulations during the admission process.
Collaboration with the Care Team:
- Work closely with physicians clinical staff and social workers to develop a care plan tailored to the patients needs.
- Serve as a liaison between the patient family and the interdisciplinary care team to ensure seamless coordination of care.
Case Management:
- Provide a smooth transition from the hospital or other care settings to hospice care.
- Coordinate the transfer of medical records including test results and treatment plans to the hospice care team.
Quality Assurance:
- Participate in quality assurance initiatives to evaluate and improve the admissions process and patient care services.
- Ensure all admission processes are conducted in accordance with hospice policies and industry best practices.
Compensation
Annual Rate: $60000- $65000 a year
