Position Summary:
A Speech Language Pathologist (SLP) will evaluate and treat residents/patients with temporary or long-term speech hearing swallowing and cognitive disabilities in compliance with state and federal guidelines. They will evaluate the residents/patients speech language cognitive-communication voice swallowing and fluency and develop treatment plans. They will treat the residents/patients based on a physicians treatment plan.
Essential Functions
- Screens and evaluates patients/residents and develops an appropriate care plan.
- Under a physicians direction provides direct patient/resident treatment based on established plan of care.
- Plans directs or conducts rehabilitative treatment programs to restore communicative efficiency of individuals with communication and/or swallowing or cognitive problems.
- Administers scores and interprets specialized hearing and speech tests.
- Instructs residents/patients families and caregivers to monitor speech and provide ways to practice new skills.
- Completes required forms and clinical records in accordance with company policy and state and/or federal regulations.
- Documents in patient records in an accurate and timely manner.
- Devises special tools and treatments for the specific needs of patients.
- Provides comprehensive and compassionate therapy services in a sub-acute/skilled nursing facility setting.
- Works collaboratively with other rehab therapists and nursing staff in a supportive team-working environment.
- Maintains resident/patient privacy pursuant to HIPAA.
- Ensures punctuality and regular attendance for assigned shifts.
Other Functions
- Develops treatment plans including short and long-term goals and methods to achieve identified goals.
- Communicates the residents/patients speech therapy program to the family and/or guardian.
- Keeps written documentation including evaluations daily notes and weekly summaries.
- Performs other duties as assigned.
Knowledge/Skills/Abilities
- Current knowledge of effective speech therapy techniques.
- Familiarity with clinical needs of geriatric population.
- Strong clinical capabilities.
Qualifications
Licenses/Certification:
Current license in Speech Language Pathology as required by state law.
