Scheduled Weekly Hours:
40Emplify Health by Gundersen Health System is looking for a full-time social worker for their adult Inpatient Rehab Unit. Opportunity to assist a variety of patient populations with discharge planning as they work towards returning to their home environment.
What you will work:
1.0 FTE 80 Hours every 2 weeks
Monday - Friday Day shifts
One holiday per year
What you will do:
Assessment and treatment planning
Discharge planning
Provide patient and family education and support

What you will need:
Bachelors degree in social work or in a related field
WI Social Worker license
1 year of previous experience in human services or health care
