Job Summary
JOB DESCRIPTION:
Provide social work services to patients in acute care maternal child care emergency department home health/ hospice setting for patients with acute chronic or terminal illness and their families. Services to patient and families include: identification of psychosocial needs develops and implements plan of care from the strengths perspective responsive to patient/family concerns health care status available resources age specific problems and clinical impressions; discharge planning crisis intervention information and referral protective services patient family education collaboration with the multidisciplinary care team and building connections with community resources.

Qualifications
JOB QUALIFICATIONS:
- Experience:
- Minimum Preferred Experience: one year experience
- Preferred Experience: 2 years experience in medical field
- Education:
- Minimum Required Education: Bachelor degree Social Work
- Preferred Education: Masters degree Social Work
- Licenses:
- Minimum Required Licenses: Licensed Bachelor of Social Work
- Preferred Licenses: Licensed Master Social Work; Licensed Specialist Clinical Social Worker
