Job Description:

Private Duty Registered Nurse

You became a nurse to lead care with purpose.

At Elara Caring you guide care where it matters most in the home. We support more than 60000 patients each day across the country. As a Private Duty RN you coordinate care support families and lead a team dedicated to one patient at a time.

On An Average Day You Will:
(includes but not limited to)

Complete comprehensive admission assessments
Determine appropriateness for home care services
Develop individualized plans of care
Coordinate with referral sources for smooth transitions home
Communicate with physicians caregivers and interdisciplinary team members
Supervise RNs LPNs and Home Health Aides
Review documentation to ensure alignment with the plan of care
Reassess and update plans of care based on patient progress
Coordinate equipment pharmacy and community resources
Ensure quality and continuity of care

What You Bring to the Team:

Graduate of an accredited RN program
Active RN license in the state of practice
Two or more years of clinical or home health experience preferred
Strong communication and organization skills
Ability to lift push pull or support up to 50 pounds
Commitment to patient centered care
Ability to work within an interdisciplinary team
