Job Description

Summary

This position is responsible for conducting participant recruitment patient navigation data entry and community engagement support for the MUSC HCC Community Outreach and Engagement (COE) Office. The work will be conducted under the supervision of the HCC COE Associate Director.

This is a community role and work is performed onsite and occasionally at some community-based activities. Evening or weekend hours and local travel may be required for outreach events.

A hybrid work schedule may be permitted after an orientation period.

Responsibilities

35% Patient Navigation

  • Provide individualized patient navigation services identify and address barriers to care such as transportation insurance financial concerns or health literacy challenges
  • Provide patient navigation services to guide the enrolled study participants to cancer screening in collaboration with the health care team to achieve targeted patient outcomes.

  • Recruit and enroll participants to ongoing projects. When needed prepare consent forms and ensure ethical standards are met.

  • Maintain regular communication with patients to ensure they schedule and receive a preventive cancer screening exam.

  • Provide patient education regarding cancer screening guidelines and available services.

  • Assist patients in understanding cancer risk screening options and followup recommendations.

  • Participate in community events workshops and educational sessions as needed.

  • Ensure compliance with organizational policies confidentiality standards and program protocols.

35%Community Outreach and Collaboration

  • Advocate for patient and family empowerment and the right to self-determination regarding their healthcare while serving as a liaison between patients providers and community partners.

  • Develop partnerships and function as a liaison with outside agencies patients the interdisciplinary team and other members of the health care system to improve the continuum of care.

  • Collaborate with program leadership community partners and clinical teams to support program goals and conduct outreach activities to increase program awareness and participation.

  • Travel when applicable to federally qualified health clinics other medical clinics churches and other community outreach sites throughout SC to provide cancer education to community residents and patients.

  • Collaborate with the program management team to refine workflows and enhance program effectiveness.

25% Quality Assurance

  • Conduct data quality assurance evaluations regularly to ensure that each database is thoroughly cleaned before it is given to a data analyst for detailed analysis. These data management/data quality assurance activities will help to ensure that accurate records are maintained.

  • Responsible for maintaining accurate timely documentation of patient interactions screening outcomes and navigation activities.

  • Assist with data collection for reporting evaluation and quality improvement initiatives.

  • Provide preliminary analysis of data related to the MUSC HCC Office of Community Outreach and Engagement projects.

  • Utilize office tools including SharePoint REDCap and Microsoft Excel

5%Education and Engagement

  • Identify and promote individual and departmental goals that align with the assigned department and hospitals strategic initiatives.

  • Identify and promote individual and departmental goals that align with Community Outreach and Engagement as well as MUSC Hollings Cancer Center strategic initiatives.

  • Identify opportunities for process improvement. Engage team members to identify optimal solutions to improve patient and community satisfaction and/or service delivery.

  • Participate in ongoing educational training and literature review to maintain the level of competence needed to perform job functions.

Preferred Knowledge Skills and Abilities:

  • Expertise in Excel. Familiarity with REDCap and other database management systems.

  • Attention to detail: ensuring accuracy in data collection and adherence to protocols is crucial.

  • Communication skills: Effective communication is essential for conveying research protocols and findings to team members and stakeholders.

  • Organizational Skills: Coordinators must manage multiple tasks and projects simultaneously and have the ability to work independently.

  • Experience in patient navigation case management or community outreach.

  • Experience working with diverse populations preferably in a healthcare or community health setting.

  • Familiarity with data tracking systems.

  • Bilingual or multilingual skills.
