Position Overview
Trinity Healthcare Talent Services is seeking a Travel Physical Therapy Assistant (PTA) for a Home Health setting in Sacramento CA.
This role involves providing in-home therapy services assisting patients in improving mobility independence and functional outcomes in their home environment.

- Contract Type: LOA Coverage (Urgent Need)
Clinical Responsibilities
Patient Care
- Provide therapy services under PT supervision
- Assist with:
- Strength and mobility training
- Gait and balance improvement
- Functional independence
- Work with patients recovering from:
- Surgery
- Chronic illness
- Mobility limitations
Home Health Duties
- Travel to patient homes
- Evaluate home environment for safety
- Educate patients & caregivers on:
- Exercises
- Equipment use
- Home safety
Clinical Skills
- Assist with:
- ROM assessments
- Therapeutic exercises
- Prosthetic training
- Functional mobility training
Documentation
- Maintain detailed records using HCHB system
- Complete:
- Progress notes
- Clinical summaries
- OASIS-related documentation
Collaboration
- Coordinate with:
- Physicians
- PTs
- Nurses & home health aides
- Participate in case conferences
Scheduling & Work Details
- 8-hour day shifts
- Full-time (40 hrs/week)
- Mileage reimbursement (GSA guidelines)
- Timesheets must be approved by supervisors
Required Qualifications
Active California PTA License
Minimum 2 years experience
Home Health experience (REQUIRED)
OASIS experience (REQUIRED)
BLS Certification
Valid drivers license auto insurance
NPI Number
Preferred Qualifications
HomeCare HomeBase (HCHB) experience
Strong communication & documentation skills
