Position Overview
Trinity Healthcare Talent Services is seeking a Travel Physical Therapist (PT) for a Home Health assignment with Heartland Home Health.
This role focuses on delivering skilled patient-centered therapy in home settings including evaluation treatment planning and ongoing care management.
Facility Information
- Facility: Heartland Home Health
- Location: West Branch MI
- Setting: Home Health
- Coverage Area: 60-mile travel radius
- Charting System: HomeCare HomeBase (HCHB)
- Caseload: 6 patients per day
- Contract Length: 13 Weeks
Key Responsibilities
- Perform in-home evaluations and assessments (including OASIS)
- Develop and implement individualized treatment plans (POC)
- Provide therapy for:
- Mobility and strength deficits
- Pain management
- Post-surgical recovery
- Cardiovascular rehab
- Deliver interventions such as:
- Gait training
- Balance & coordination training
- ROM assessment (goniometer use)
- Electrical stimulation ultrasound heat therapy
- Educate patients caregivers and family members
- Maintain accurate documentation in HCHB EMR
- Supervise PTA CNA and Home Health Aides
- Participate in interdisciplinary team meetings
Clinical Highlights
- Home Health setting (independent work)
- 6 patients/day (manageable caseload)
- No call requirement
- Mix of rehab functional restoration care

- No weekends / no call
Required Qualifications
Active Michigan PT License
Home Health experience (MANDATORY)
HCHB experience (REQUIRED)
Valid drivers license auto insurance
Degree in Physical Therapy (Bachelors/Masters/DPT)
Preferred Qualifications
OASIS documentation experience
Cardiovascular rehab experience
Strong patient education & communication skills
Compliance Notes
Must be comfortable traveling within 60-mile radius
Independent field work required
Must have reliable transportation
