Trinity Healthcare Talent Services is seeking a Travel Physical Therapist (PT) for a Home Health assignment with Mercy Home Health in Sacramento CA.
This role involves delivering in-home physical therapy services focusing on patient recovery mobility improvement and independence in a real-world environment.

Clinical Responsibilities
- Perform in-home patient evaluations and assessments
- Develop and implement personalized treatment plans
- Provide gait training mobility and strength rehabilitation
- Assess home environment for safety and functional needs
- Educate patients and caregivers
- Monitor progress and communicate with physicians
- Maintain accurate documentation using HCHB EMR

Required Qualifications
Active California PT License
BLS Certification
OASIS Experience REQUIRED
Minimum 2 years healthcare experience

Strong documentation and communication skills
Scheduling & Assignment Details
- Schedule: 5x8 Days
- Hours: 40 hours/week
- Weekend: Every other weekend (EOW)
- On-call: Not specified
Important Notes
Must have OASIS Home Health experience
Requires 2 supervisor references at submission
Mileage must be tracked and approved
Cannot have worked with CommonSpirit in past 1 year
