Licensed Practical Nurse (LPN) / Licensed Vocational Nurse (LVN) Hourly

What Youll Do

Provide direct patient care based on the Plan of Care and physician orders
Support patient goals through skilled nursing visits
Monitor patient condition and report changes to the RN Case Manager and care team
Educate patients and families on self-care and safety
Document care medications and updates accurately
Collaborate with the RN Case Manager and clinical team
Help prevent hospitalizations through proactive care

What You Need

Graduate of an accredited nursing program
Active LPN/LVN license
Minimum 1 year of clinical experience
Home health or hospice experience preferred
Ability to travel within the coverage area (up to 50%)
Ability to lift push pull or support up to 50 pounds
Valid drivers license auto insurance and reliable transportation
