Our patient census is expanding. Our team is growing. We are hiring a Clinical Team Manager (RN) to help lead and support our home health clinicians as we continue serving more patients in the community.

What you will do

Oversee daily patient care services and clinical staff
Assign clinicians and coordinate patient visits and referrals
Ensure patient needs are assessed and care plans are developed and updated
Coordinate care with physicians vendors and care teams
Review and approve plans of care and physician orders
Conduct OASIS reviews and ensure regulatory compliance
Coach and mentor field clinicians
Participate in interviewing and hiring clinical staff
Monitor branch clinical performance and outcomes
Support field visits when needed

What you need

Associate degree in Nursing or higher
Active RN license in Indiana
2 years home health or hospice RN experience
1 year leadership experience preferred
Knowledge of Homecare Homebase and Microsoft Office preferred
Reliable transportation valid drivers license and current auto insurance
Ability to lift push pull or support up to 50 pounds

You will report to the Branch Director and play a key role in supporting our expanding clinical team and delivering high quality care to patients across the Valparaiso community.
