Unit Overview
We are seeking a dedicated and compassionate Family Practice Provider to join our Cooper Care Alliance team. The ideal candidate will provide comprehensive patient-centered care to individuals and families across all ages focusing on prevention wellness and the management of chronic and acute conditions in an outpatient setting.
Short Description
The Advanced Practice Nurse assumes responsibility for advanced nursing practice within a clinical consultation with other members of the health care team strengthens nursing practice and promotes positive patient outcomes. Responsibilities include clinical practice staff and patient education and research. The Advanced Practice Nurse serves as a significant member of the multidisciplinary health care team in the inpatient outpatient and community settings.
Key Responsibilities:
- Provide high-quality evidence-based primary care to a diverse patient population.
- Conduct physical exams diagnose and treat illnesses order and interpret diagnostic tests.
- Develop and manage treatment plans including prescribing medications and coordinating referrals.
- Promote preventive care and health education to patients and families.
- Collaborate with interdisciplinary teams including nurses medical assistants behavioral health and specialists.
- Maintain accurate and timely documentation in the electronic health record (EHR).
- Participate in quality improvement initiatives and clinical meetings.
- Adhere to organizational policies procedures and regulatory standards.
Education Requirements
Graduate from masters level program.
Graduate from an NLN approved school of nursing.
Experience Required
Experience with electronic health records (e.g. Epic).
Experience in an ambulatory or outpatient care setting preferred.
License/Certification Requirements
Obtain and maintain certification by the NJ State Board of Nursing as Advanced Practice Nurse.
Current BLS and ACLS certification National certification within area if specialty preferred

- Strong communication organizational and interpersonal skills.
- Commitment to providing culturally competent and equitable care.
- Bilingual or multilingual skills are a plus.
- Interest in population health quality improvement or teaching.
