- Salary: $135004 $202555 (based on experience)

Job Summary:
We are seeking a highly experienced Director of Case Management to lead the strategic direction and daily operations of the Case Management department including Social Work and Chaplain services within a large acute care hospital. This role is responsible for ensuring effective care coordination discharge planning regulatory compliance and optimal utilization of healthcare resources to improve patient outcomes.
Responsibilities:
- Provide leadership and oversight of all Case Management operations including staffing workflows and care coordination models
- Ensure compliance with regulatory standards organizational policies and industry best practices
- Lead discharge planning and continuity of care initiatives across the organization
- Analyze departmental performance data and implement process improvement strategies
- Collaborate with executive leadership and external stakeholders to align operational and clinical goals
- Manage departmental resources including staffing models (FTEs) and productivity targets
- Build and maintain strong relationships with physicians and post-acute care partners
- Lead mentor and develop a leadership team including managers and supervisors
- Oversee a large team (approximately 110 FTEs) across Case Management Social Work and Chaplain services
- Support organizational mission values and compliance standards
Qualifications:
Education:
- Bachelors Degree (required)
- Masters Degree in Nursing Healthcare Administration or Business (preferred)
Licensure:
- Active RN license or LCSW/LMSW (required)
Experience:
- Minimum 3 years of acute care hospital Case Management experience
- Minimum 2 years of leadership experience in Case Management
- Strong experience in data analysis strategic planning and performance improvement

- Strong communication and stakeholder engagement skills
- Expertise in care coordination discharge planning and utilization management
