Job Summary:
We are seeking a compassionate Certified Nursing Assistant (CNA) to support a Hospital at Home program delivering acute care services in patients homes. The CNA will work alongside healthcare providers to ensure high-quality patient-centered care outside of the traditional hospital setting.
Key Responsibilities:
- Assist with daily patient care in a home-based setting
- Support providers during in-home patient visits
- Monitor and record vital signs and patient status
- Provide hygiene and comfort care
- Assist with mobility and daily living activities
- Maintain accurate and timely documentation
- Communicate patient updates to the care team
Key Requirements:
- Active New York State CNA License
- Minimum 1 year of CNA experience
- High School Diploma or equivalent
- Strong communication and patient care skills
