Job Description
HealthX is seeking an experienced and compassionate Clinical Care Coordinator to join a well-established residential aged care facility in Port Macquarie NSW. This is a key leadership role within a 105-bed service where you will support the delivery of high-quality person-centred care while ensuring compliance with regulatory standards.
Reporting directly to the Facility Manager you will provide hands-on clinical leadership guide nursing and care teams and drive best-practice outcomes for residents.
Key Responsibilities
Clinical Leadership & Governance
Provide clinical leadership and oversight across all care services
Ensure compliance with Aged Care Quality Standards and relevant legislation
Lead clinical audits risk management and continuous improvement initiatives
Support accreditation readiness and ongoing compliance
Care Planning & Clinical Oversight
Oversee resident assessments care planning and evaluations
Manage complex clinical needs including wound care dementia palliative care and chronic conditions
Ensure best-practice medication management and clinical documentation
Review incidents and implement preventative strategies
Team Leadership & Support
Supervise mentor and support RNs ENs and care staff
Assist with rostering to ensure safe staffing levels
Support recruitment onboarding and staff development
Foster a positive and collaborative team culture
Communication & Stakeholder Engagement
Liaise with residents families GPs and allied health professionals
Communicate clinical information clearly and compassionately
Advocate for resident needs preferences and wellbeing
About You
To be successful in this role you will have:
Current AHPRA registration as a Registered Nurse (Division 1)
Proven experience in a senior clinical role within residential aged care
Strong knowledge of Aged Care Quality Standards and clinical governance
Demonstrated leadership and team management skills
Excellent communication and documentation abilities
Experience managing complex clinical cases in medium-to-large facilities
Proficiency with aged care clinical systems
