Key Responsibilities
- Provide direct patient care in accordance with established care plans
- Monitor and document patient conditions and report changes to the care team
- Administer medications and treatments as prescribed
- Assist in the development and implementation of patient care plans
- Maintain accurate and timely documentation
- Collaborate effectively with RNs physicians and other healthcare staff
Requirements
- Active Licensed Practical Nurse (LPN) license in the state of Delaware
- Prior experience in long-term care rehabilitation or skilled nursing preferred
- Strong clinical communication and organizational skills
- Ability to work effectively in a team-oriented environment
