Your Opportunity

Are you passionate about making a real difference in the lives of young people and families at risk of homelessness Join our dynamic team as a Youth Practitioner where youll play a crucial role in providing support guidance and hope to vulnerable youth and their families.

In this exciting and rewarding role youll offer personalized case management family support and lead group work programs to help prevent homelessness and build brighter futures. Youll work closely with young people to understand their unique needs create tailored action plans and connect them with the services and resources they need to thrive.

Youll be the go-to person for providing essential information advice and referrals while also building strong supportive relationships with families to help strengthen their foundations. Youll be part of a passionate collaborative team thats committed to creating lasting change and empowering young people to overcome challenges and reach their potential.

Your key responsibilities will be to:

  • Provide intensive ongoing person-centred case management
  • Assist clients through brief therapeutic intervention case management advice information and referral
  • Conduct group case management sessions for clients where necessary and appropriate
  • Assist Program Manager and other employees in addressing complex cases or emergency situations
  • Complete a range of administrative duties for the efficient running of the service
  • Build and maintain positive internal and external relationships

Requirements for Success

  • Hold a relevant degree and at least 1 years experience in a similar role
  • Demonstrated professional approach to the role including the ability to maintain professional boundaries
  • Case management case co-ordination strong collaboration skills
  • Demonstrated ability to provide specialised therapeutic interventions and manage a caseload of complex clients
  • Demonstrated ability to apply basic counselling and mediation frameworks to intervention young people and their families
  • A current and valid Drivers licence

Before starting work with us you will need to undertake a national police history check qualifications and referee checks and have a clearance to work with children/vulnerable people.
