Job Summary
Synergy Care is looking for a qualified Licensed Physical Therapy Assistant for an open position at an Independent Living Community in Oklahoma City OK. We are looking for someone who will provide quality patient care and be a team player.
This position offers Independent Assisted/Memory Care and SNF.
Job responsibilities:
Provides physical therapy and related services for patients caregivers and facilities in accordance with the principles and practices of occupational therapy and with Synergy Management Solutions guidelines. Provides quality PT including: Evaluation of patients development of initial and ongoing treatment & discharge plan supervision of assistants and aides & participates in case management. Represents Synergy Management Solutions commitment to quality & service in all interactions with facility.

In order to be considered for this position you must be a graduate of an accredited school for Physical Therapy/Physical Therapist Assistant or will graduate from such an institution within the next six (6) months.
