What Youll Do
As a PRN SLP youll deliver personalized one-on-one care in the home setting helping patients regain and maintain essential communication and swallowing functions.
Evaluate patients and develop individualized treatment plans
Perform OASIS-based assessments and ongoing re-evaluations
Provide evidence-based speech-language therapy in the home
Educate patients and caregivers on therapy techniques and goals
Collaborate with physicians nurses and interdisciplinary team members
Adjust plans of care based on patient progress and needs
Maintain timely accurate clinical documentation

Requirements
Masters Degree in Speech-Language Pathology
Current active SLP license in Michigan
Minimum of 1 year clinical experience (home health preferred not required)
Reliable transportation valid drivers license and auto insurance
Willingness to travel within the Ludington/Big Rapids coverage area
Ability to perform physical tasks (lifting positioning patients etc.)
