The Stride Connector Hub works to strengthen the ability of people with psychosocial disability to participate more fully in the community and enhance their capacity for social and economic participation healthcare.

Role Overview:

The purpose of this role is to deliver high quality stepped care services in the community to people who experience severe and persistent mental illness and their carers.

Responsibilities:

  • Participation and input into developing culturally friendly practices especially when working with First Nations people.
  • To be a point of contact for First Nations Communities in our area and to help to further strengthen our relationships with these communities.
  • Conduct assessments of individuals needs presenting to the service and develop a Recovery Care Plan that addresses these needs
  • Conduct risk assessments including assessment of self-harm and suicide risk violence/aggression and vulnerability in collaboration with the individual their family/carers and other support systems with active engagement of appropriate referrals and treatment planning
  • Strengthening their ability to coordinate and implement supports and identifying services in line with their stated goals and desired outcomes
  • Liaise with and develop collaborative working relations with key service providers to ensure ease of access to mental health and other services e.g. social services housing
  • Facilitate and monitor the integration and coordination of care of individuals including participating in case review meetings
  • Participating in groups and 1:1 supports
  • Assist clients to assess against eligibility for the NDIS


Requirements:

  • It is an essential requirement to identify as an Aboriginal and/or Torres Strait Islander person
  • Minimum of a Cert IV in a Community services related discipline (ideally a completed degree or Diploma)
  • Experience in mental health assessment and subsequent coordination of access to appropriate service provision
  • Knowledge of and experience in the provision of mental health services to individuals and their families including knowledge of contemporary evidence-based interventions
  • Ability to develop and deliver individual or group-based skills training activities
  • Highly developed communication (including written) and interpersonal skills and the ability to engage with individuals advocate on their behalf and establish and maintain effective relationships with staff clients carers and stakeholders
  • Demonstrated knowledge of the NDIS and experience supporting individuals to test their eligibility
  • Good computer skills
  • Must be eligible to apply/currently hold a WWCC
  • Must hold/be willing to apply for NDIS worker screening clearance
  • Current Drivers License
