Were seeking a compassionate and skilled Physical Therapist to join our team at Providence Medical Foundation. As a Physical Therapist with our team youll work closely with our patients Primary Care Providers and specialists to provide the highest level of care and ensure a smooth transition to the care they need.

Weve been serving our communities for over 8 years and we welcome patients of all ages and cultures. Our staff is diverse and speaks multiple languages including Spanish and Hindi. We specialize in orthopedic neurological and cardiac conditions as well as pre and post-surgical orthopedic conditions.

Required qualifications:
- Bachelors Degree
- Graduate of an accredited Physical Therapy professional program.
- California Physical Therapist License upon hire.
- National Provider BLS - American Heart Association upon hire.
- New Grad or entry level experience within an ambulatory care facility or medical group practice.
- Experience with program planning implementation evaluation and education training.
Preferred qualifications:
- Masters Degree or Ph.D.
- 1 year of Experience in outpatient Physical Therapy setting.
- Experience with CARF and Title XXII standards.
- EMR experience.
Graduate of an accredited Physical Therapy professional program.
1 year of Experience in outpatient Physical Therapy setting.
California Physical Therapist License upon hire.
Experience with CARF and Title XXII standards.
Experience with program planning implementation evaluation and education training.
