ADMISSIONS AND DISCHARGE PLANNING

Committed to the healing of body mind and spirit the social worker conducts psychosocial assessments provides education and therapeutic interventions for patients and families as appropriate to achieve an optimal level of care.
HOURS: Monday-Friday daytime hours.
SKILLS KNOWLEDGE EDUCATION AND EXPERIENCE:
Licensed as one of the following in the state where practicing:
Licensed Clinical Professional Counselor (LCPC)
Licensed Master Social Worker (LMSW)
Licensed Clinical Social Worker (LCSW)
Licensed Professional Counselor (LPC)
Licensed Marriage and Family Therapist (LMFT)
Two years of post-license experience in related mental health setting preferred
Case management and discharge planning experience essential.
Basic Life Support for Healthcare providers certification from either AHA ARC or the Military Training Network is required
ESSENTIAL FUNCTIONS:
Know understand incorporate and demonstrate the Organizations Mission Vision and Values in behaviors practices and decisions
Primary responsibilities will include case management and discharge planning.
Assess patients treatment needs and provides appropriate interventions including but not limited to the following:
group and family therapy
risk assessment
chemical dependency groups
case management with community providers
facilitating and documenting a viable discharge plan
Complete psychosocial assessment and consult with physicians and hospital staff regarding patient and family psychological issues through formal and/or informal multidisciplinary conferences and treatment planning. May be called to facilitate the Treatment Planning process
Work cooperatively with immediate team and other hospital caregivers to meet the emotional mental and spiritual needs of patients and/or their family/caregivers
Complete clinical documentation in required timeframe and within set standards
Has experience in hospital admission and discharges
Experience with case management- for both adult and pediatrics
Familiar with community resources- for both adults and pediatrics
Possesses an understanding of Medicaid and Medicare
Possesses an understanding of the Idaho mental health hold and commitment procedures
Has experience running TX teams
Passion for mental health and supporting patients with after care
