Job Summary:
We are seeking an experienced Director of Case Management to lead and oversee all case management operations within a hospital setting. This role is responsible for ensuring effective care coordination discharge planning regulatory compliance and optimal patient outcomes. The ideal candidate will collaborate closely with interdisciplinary teams and hospital leadership to drive operational efficiency and quality improvement initiatives.
Responsibilities:
- Lead and manage daily case management operations.
- Oversee care coordination and discharge planning processes.
- Ensure compliance with hospital policies procedures and regulatory standards.
- Manage staffing performance and departmental productivity.
- Analyze operational data to monitor and improve performance.
- Collaborate with hospital leadership (CFO CMO CNO and other stakeholders).
- Build and maintain strong relationships with internal teams and external partners.
- Drive quality improvement initiatives and enhance patient care outcomes.
Required Qualifications:
Education:
- Bachelors Degree (required)
- Masters Degree in Nursing Healthcare Administration or Business (preferred)
Licensure:
- Active RN License OR LCSW/LMSW License (required)
Experience:
- Minimum 3 years of acute care hospital case management experience (required)
- Minimum 2 years of leadership experience in case management (required)
- Recent acute care experience required
