As an integral member of the IMPACT team the Peer Support Worker works collaboratively with members of the multi-disciplinary team to provide a service to patients with severe mental health challenges. The principal responsibilities include: providing support to IMPACT clients in meeting their treatment and rehabilitation goals; facilitating client social and recreational opportunities; providing a valuable perspective for service users within the Assertive Community Treatment (ACT) model and the Mental Health System; participating in the training and supervision of students; performing cross-functional and/or other duties as assigned or requested to contribute to the efficient and effective provision of service.
- Provide supportive client-centered care within the ACT model including visiting clients in the community helping maintain a safe environment and connecting clients with community resources and services.
- Advocate for clients to help them access the supports they need and provide education to clients and families about their rights and how to access and maintain services.
- Helps connect clients with social and recreational opportunities that match their interests and needs including accompanying them to appointments activities and community events when appropriate.
- Supports and encourages client participation in social and recreational activities by helping plan coordinate and explore different opportunities and by sharing information about available events and programs
- Draws upon lived experience to provide invaluable knowledge to staff students and the community at large including imparting knowledge of different/appropriate types of techniques i.e. listening questioning etc.
Qualifications :
- At minimum a completion of a high school diploma
- Completion of recognized Human Services or Social Services diploma is an asset
- Two years work experience in Mental Health and/or Substance Use Preferred
- Lived experience with Mental Health challenges and/or Substance Use
- Peer Support Certificate or training is an asset
