Job Summary
Job Title: LPN / LVN Long Term Care
Location: New York
Schedule
- Day Shift: 6:30 AM 2:30 PM
- 37.5 hours guaranteed per week
- Every other weekend required
Job Summary
The LPN/LVN Long Term Care provides direct patient care to residents in a skilled nursing facility. This role involves supporting patients with chronic conditions assisting with daily care needs and collaborating with the healthcare team to ensure high-quality compassionate care.
Key Responsibilities
Patient Care
- Provide direct nursing care to residents under the supervision of an RN
- Assist with Activities of Daily Living (ADLs) including bathing dressing and mobility
- Monitor patient conditions and report changes promptly
- Administer medications and treatments within scope of practice
Clinical Support
- Support care for patients with conditions such as MS orthopedic cases dementia and memory care needs
- Assist with care planning and implementation
- Float to different units within the facility as required
Documentation & Systems
- Maintain accurate and timely patient records
- Utilize EMR systems (e.g. PointClick Care)
Compliance & Safety
- Follow facility policies and regulatory standards
- Ensure infection control and patient safety protocols are maintained
Required Qualifications
- Active LPN/LVN license (New York)
- Minimum 1 year of experience in long-term care or similar setting
- BLS (Basic Life Support) certification
