Job Summary
CVHCare is looking for a qualified Physical Therapist. Assigned county Territory is Area #12 (San Jose) and Area #11 (Santa Clara Sunnyvale Cupertino Milpitas Mt. View Campbell Los Altos Saratoga Los Gatos) Additional travel outside area may be required. Weekend coverage may be required. If Guarantee the following shall apply: Productivity: At least 27 points per week (points described below). 36 hours Guarantee. Traveler must complete 27 points in the week. Must be willing to travel outside the area or work weekends if required to maintain weekly visit expectations. If Traveler refuses patients guarantee does not apply. Guarantee does not apply for 1st and last week of assignment or for weeks in which there are holidays or for weeks in which the traveler has time off. If Non-Guarantee the following shall apply: To secure a full caseload Traveler may be required to go outside the service area occasionally. Can refuse patients. No guaranteed hours. Traveler must be willing to see Covid and Covid Pui patients. Payment made on per visit basis. See below for visit types and how they convert to points and how they convert to hours. This is how we will calculate how many hours are approved per day. OASIS ReCertification and OASIS RoC Routine Eval Discharge Eval SoC Eval RoC Eval ReCert Eval F/up Re-Eval Eval Discharge Oasis Discharge Oasis F/up 1 point 1.33 hours. OASIS SoC 1.5 point 2 hours. Although Traveler s are paid hourly by their agency hours are approved by CVHCare on a per visit bases. Note that Rate is all inclusive and covers drive/travel time visit time documentation time & time spent in making phone calls to patients & doctors. Credentials: Current CA PT License BLS Drivers License Auto ins Auto Reg TB (annual) MMR Varicella HepB TDap (10 years) Flu (annual) Drug screen Physical. Your Agency s contract to your traveler MUST mirror the confirmation page. If your agency does not comply with this standard your agency will be at risk of being removed from this client s vendor set. No exceptions! Start Date Must be Identical. End Date Must be Identical. ***Please Note - All Requested Time Off (RTO) must be disclosed at the time of submission. If your agency or the traveler does not include all RTO in the submission form and adds it post confirmation your agency will be at risk of being removed from this client s vendor set. No exceptions! Time off requested after the acceptance date will not be approved.

additionalInformation: Former employees are not eligible for travel assignments. Agency employee must work at least 3 scheduled shifts after completion of orientation for Agency to be paid for orientation hours. Orientation is paid at $10 less per hour than the all-inclusive agreed upon hourly rate. Overtime is paid at $10 more per hour than the all-inclusive agreed upon hourly rate. Overtime is paid after 40 hours (30 RVEs) per week. All approved worked hours will be calculated based on the below Point Value/Hourly conversion table below. Routine Eval Discharge Eval SoC Eval RoC Eval ReCert Eval F/up Re-Eval Eval Discharge Oasis Discharge Oasis F/up 1 point 1.33 hours. OASIS SoC and OASIS ReCertification and OASIS RoC 1.5 point 2 hours. Note that Rate is all inclusive and covers drive/travel time visit time documentation time & time spent in making phone calls to patients & doctors. Agency employee will be issued a Smartphone and medical devices (stethoscope pulse ox gait belt blood pressure cuff etc). Any lost stolen or damaged equipment will be the sole responsibility of the Agency to fully reimburse the Facility. Facility data plan is optimal for full time field clinicians (documentation texting calls hotspot). Data usage will be kept to a strict minimum reasonable and necessary for business. Facility has established a monthly max limit of 3 GB per user. Any overage charges incurred will be the sole responsibility of the Agency to fully reimburse the Facility.
