Schedule: Outpatient Monday Friday with variable start times between 7:00 AM 6:00 PM

Job Summary:
The Physical Therapist evaluates plans and delivers evidence-based physical therapy interventions to patients across outpatient and related care settings. This role focuses on improving mobility strength pain management and overall functional independence through individualized treatment plans. The PT collaborates with interdisciplinary teams supports discharge planning and educates patients and caregivers on home exercise programs and safety strategies while ensuring compliance with clinical and regulatory standards.

Key Responsibilities:

Conduct comprehensive patient assessments
Develop and implement individualized treatment plans
Provide evidence-based physical therapy interventions
Improve patient mobility strength and functional independence
Collaborate with interdisciplinary healthcare teams
Participate in discharge planning
Educate patients and caregivers on home exercise programs and safety
Maintain accurate and timely clinical documentation
Ensure compliance with facility and regulatory standards
Perform additional duties as assigned

Qualifications:

Graduate of an accredited Physical Therapy program
Active Physical Therapy (PT) license in the state of practice
Basic Life Support (BLS) certification required
Rehabilitation or Acute Care experience preferred
