Position Overview
Trinity Healthcare Talent Services is seeking a Travel Physical Therapist (PT) for a home health setting serving the Franklin/Macon County area near Asheville NC.
This role involves providing in-home rehabilitation services to patients recovering from illness injury or surgery. The Physical Therapist will focus on improving mobility restoring independence and delivering patient-centered care through individualized treatment plans.
The therapist will work independently in a field-based environment while collaborating with interdisciplinary teams to ensure optimal patient outcomes.

Clinical Coverage Areas
Therapists will provide care for:
Post-surgical recovery
Fall-related injuries
Mobility and functional impairments
Chronic condition management
Home health patients requiring rehabilitation services
Key Responsibilities
Perform in-home patient evaluations and functional assessments
Develop individualized treatment plans based on patient needs
Deliver therapeutic exercises mobility training and rehabilitation interventions
Manage a caseload of approximately 56 patients per day
Monitor patient progress and adjust treatment plans accordingly
Educate patients and caregivers on home exercise programs and safety
Maintain accurate and timely clinical documentation (HCHB)
Coordinate care with interdisciplinary healthcare teams
Manage daily travel and scheduling efficiently
Required Qualifications
Minimum 1 year of Home Health Physical Therapist experience
Active North Carolina Physical Therapist License
Strong communication and documentation skills
Reliable personal vehicle for patient visits
Preferred Qualifications
Experience with HomeCare HomeBase (HCHB)
Previous travel experience
Ability to work independently in a field-based setting
Scheduling & Assignment Details
40 hours guaranteed per week
Monday Friday schedule
Shift: 8:00 AM 5:00 PM
