Position Overview:
This entry-level position works under the general direction of the Day Center Lead. Responsibilities include providing participants with assistance in personal care supporting center activities and assisting with meal service. The Certified Nursing Assistant is an integral member of the PACE team contributing to a safe supportive and engaging environment for participants in the center setting.
Key Responsibilities
- Assist participants with basic activities of daily living including but not limited to bathing dressing incontinent care toileting and other assigned tasks.
- Assist participants with transfers and mobility with or without assistive devices ensuring safety at all times.
- Assist with oral feedings of participants as needed following safety protocols.
- Assist with center activities and restorative therapies as directed to support participant engagement and well-being.
- Serve snacks and meals in accordance with safety standards; obtain Serve Safe Handler certification.
- Participate in required training in-service education programs and center-based meetings to maintain and enhance skills.
- Assist in maintaining the cleanliness and organization of the facility to ensure a safe environment.
- Assist in the transportation of participants for facility outings as needed.
- Observe and report any changes in a participants condition to the Day Center Lead in a timely manner.
- Communicate effectively with PACE team members including interdisciplinary staff to support coordinated care.
- Assist participants in their homes with personal care needs on an as-needed basis.
- Complete other tasks as assigned by the supervisor.
Qualifications & Requirements
- Education: High School Diploma or GED required.
- Certifications: CPR certification required. Current Certified Nursing Assistant (CNA) License preferred.
- Experience: Minimum one year of experience with older adults required.
- Skills: Strong interpersonal skills with the ability to handle unusual or crisis situations calmly and effectively. Ability to communicate clearly with center staff participants caregivers and family members.

- Perform participant transfers regularly with or without the use of mechanical devices as indicated.
- Work with participants with varying cognitive levels physical and sensory impairments limited social supports declining health and at end-of-life.
- Communicate and interact with PACE center staff participants caregivers and family members on a daily basis.
- Prepare for the possibility of unexpected crisis situations to occur.
Physical Requirements:
Must be able to lift and carry up to 15 pounds. Position requires standing less than 33% of time on duty walking less than 33% of time on duty and sitting between 33% and 66% of time on duty. Use of hands to finger handle or feel over 66% of time on duty. Reaching with hands or arms less than 33% of time. Full ability to talk and hear. Correctable vision required.
