The Department
The Hospital Admissions Risk Program (HARP) is a Health Independence Program (HIP) and sits within Alfred Healths Home Acute and Community (HAC) Program. As defined by the Department of Health and Human Services (DHHS) HARP aims to provide hospital substitution and diversion by supporting people in the community in ambulatory settings and in peoples homes.
Alfred Healths HARP service is delivered by a multidisciplinary workforce operating within an interdisciplinary model of care.
We are currently seeking an experienced Social Worker with a strong interest in chronic and complex holistic health care. The ideal candidate will have demonstrated experience in coordinating and supporting individuals with complex psychosocial needs as well as leading and mentoring clinicians within a multidisciplinary team.
The Position
The HARP Senior Clinician - Social Work plays a key role in providing clinical leadership and coordinating service delivery within the centralised HARP Allied Health team. The Senior Clinician - Social work provides professional leadership/supervision of the Case Management/Social Worker unit (currently 4 Social Workers) and contributes to operations and activity of that unit. As a leader within the HARP AHP cohort they may be expected to support physiotherapists occupational therapists and dietitians. The Senior Social worker will also hold a small caseload of their own in-line with their EFT.
