Your new role

Youll be part of a dynamic team delivering a wide range of programs across HACC Support for Carers and other community initiatives. We work flexibly creatively and collaboratively to support people experiencing changing or emerging needs always with a strong focus on inclusion access and community connection.

In this role youll provide personcentred earlyintervention support to individuals carers and families. Your days might include:

• Conducting assessments and offering targeted shortterm support
• Goalbased planning and coaching
• Linking clients with specialist services
• Delivering group programs and practical daytoday assistance
• Supporting carers to stay connected confident and resilient

Youre someone who thrives when supporting others and you bring warmth initiative and sound judgement to every interaction. You will enjoy this role if you are:

Your skills and experience

• A relevant qualification in Social Work Community Services Psychology Counselling or a related discipline or demonstrated equivalent knowledge and experience relevant to community services practice.
• Experience providing short-term goal-focused early intervention support to consumers with emerging needs.
