Operating from a trauma-informed client-centered holistic perspective the role addresses the needs of individuals experiencing chronic and episodic homelessness mental illness addiction and other stability challenges. This includes ensuring basic needs and referrals for individuals sleeping rough through street-based interventions. Diverse client needs are met through integrated case management. The supervisor maintains a small caseload of clients with complex concurrent disorders and supports social workers in treatment planning navigation and resource provision. The Community Outreach Supervisor is dedicated to innovative therapeutic approaches for street-involved individuals recovery and wellness. Collaboration with internal and external stakeholders through committee meetings tours and presentations ensures effective care and service delivery.

MAJOR DUTIES AND RESPONSIBILITIES
- Provideâleadership and guidance to the outreach team including supervision training and support for team members;
- Work collaboratively within the multidisciplinary team to provide comprehensive case management to clients as needed;
- Manage two distinct funding streams to ensure financial stability and sustainability of the program;
- Review funding requests to ensure eligibility and compliance with contracts;
- Prepare and submit regular reporting to internal and external stakeholders;
- Ensure the program maintains up-to-date data collection practices and provides necessary support as needed;
- Provide ongoing assessment and counselling support to a small case load of clients struggling with mental health and substance dependency needs followed by referrals to receive diagnosis and treatment;
- Provide assistance and guidance to the SEEDReach staff in managing their caseloads by facilitating weekly case consultation meetings;
- Helps facilitate the clients optimized utilization of all available community supports;
- Document client interactions client referrals and client progress on the Client Care and Management system (CCMS) and the clinics electronic medical record (EMR);
- Facilitate emergency interventions as needed;
- Efficiently manages team schedules and client appointmentsâoptimizingâservice delivery;
- Assistâin the planning and execution of ID Clinics ensuring clients identification needs are met;
- Contribute to the development and enhancement of outreach programs to meet the evolving needs of clients;
- Support in the development and facilitation of group sessions and workshops aimed at addressing mental health and addiction (Dialectical Behavioural Therapy Skills);
- Assists the Health and Wellness Manager with new initiatives or projects as required;
- Engage in leadership committee and other relevant meetings to contribute insights and strategies related to programming and remain involved in community initiatives;
- Foster productive partnerships and collaborations with community organizations government agencies and other stakeholders to enhance program effectiveness;
- Identifyâgrant opportunities prepare grant applications and secure funding to sustain and expand program servicesâ;
- Work with Health and Wellness Manager to maintain develop and evaluate Health and Wellness policies as it pertains to the mobile outreach team;
- Stay updated on current and informed practices in the social work homeless and outreach sector to ensure team members receive necessary trainingsâ;
- Provide collaboration consultation and support to additional internal teams such as the shelter;
- Provide input into program evaluation upon request.
MINIMUM QUALIFICATIONS AND SKILLS
- A passion to be part of ending homelessness in Alberta;
- Bachelors of Social Work (BSW) Masters Degree Social Work (MSW) or equivalent with active registration in good standing;
- Minimum four years experience working with vulnerable populations and homelessness;
- Proven record of mental health assessment counseling and ability to implement a plan of care;
- Demonstrated excellence in written oral and interpersonal communication with the ability to effectively engage clients multidisciplinary team members and community agencies;
- Demonstrated knowledge of Calgarys community (mental) health and addiction resources and adept in navigating those systems;
- Knowledge of FOIP and ability to observe confidentiality standards;
- A self-starter with the ability to work in a cross-functional team and accomplish goals;
- Proficient computer skills (Microsoft Office Programs);
- Experience with Homeless Management Information System (HMIS) an asset;
- Good organizational time management and advocacy skills;
- Possession of a valid Drivers License (Class 5) and a clean drivers abstract.
WORKING CONDITIONS
- This position requires a blend of office work and direct street outreach activities;
- This position may entitle working in extreme weather conditions. Being aware of the daily weather report is essential;
- Lifting travelling sitting standing walking driving personal vehicle are required as part of this role;
- Moderate physical effort required to successfully accomplish the essential job functions such as entering data into databases and responding to a fast-paced work environment;
- Moderate mental effort required to successfully accomplish the essential job functions such as counseling referrals training and responding to sensitive and or highly intense situations;
