Job Summary

POSITION SUMMARY The Occupational Therapist position treats injured ill or disabled patients through the therapeutic use of everyday activities and help these patients develop recover improve as well as maintain the skills needed for daily living and working. ESSENTIAL JOB DUTIES Evaluate and treat patients in the acute care and nursing home settings. Also screen patients and attend meetings regarding patient progress. Documentation of evaluation progress notes daily notes insurance and discharge notes in patients medical records. Collaborate with team members to ensure effective carryover of services for patient care. Outpatient evaluation and treatments. Perform other duties as requested. REQUIRED QUALIFICATIONS Required qualifications to effectively perform job. An equivalent combination of education training and experience will be considered. Master of Occupational Therapy required. Doctorate preferred. National Board Certification in Occupational Therapy and Wisconsin occupational therapy license and continuing education to maintain licensure. Must maintain a valid current driver s license. BLS certification required. 1 years of related work experience required. Exceptional accuracy and attention to detail required.

Shift: Monday-Friday 7:30am-4pm no weekends holiday rotation
