The MDS COORDINATOR/UNIT MANAGER oversees the patient care assessment and care plan process of each resident in accordance with all laws regulations and Company standards. Reports to Director of Nursing (DON).

ESSENTIAL FUNCTIONS:

  • Oversee and coordinate the care of our residents
  • Auditing patient charts for compliance
  • Attending IDT meetings
  • Attending and preparing for compliance meetings
  • On Call rotations
  • Must Possess a current unencumbered RN license.
  • Must have 3 years of RN/LPN experience and 1 year of supervisory experience.
  • MDS experience preferred.
  • Resident assessment coordinator (RAC) must be completed within 6 months of hire. RAC certification is preferred.
  • Must be knowledgeable of nursing practices and procedures as well as the laws regulations and guidelines governing nursing functions in the long-term care facility.
  • Must possess leadership ability and willingness to work harmoniously with and supervise professional and non-professional personnel.
  • Must have the ability to implement the programs goals objectives policies and procedures of the Nursing department.
  • Must be able to read current literature talk with colleagues continue education or participate in professional organizations or conferences to keep abreast of developments in the field.
  • Must be able to relate information concerning residents conditions.
  • Must be able to investigate and direct resident inquiries or complaints to appropriate medical staff members and follow up to ensure satisfactory resolution.
  • Must be able to collect and report data on topics such as resident encounters and inter-institutional problems making recommendations for change when appropriate.
  • Must be able to coordinate communication between residents family members medical staff administrative staff or regulatory agencies.
  • Must understand and follow company policies including harassment and complaint procedures.
  • Must be able to coordinate the timely development and completion of resident assessment (MDS).
  • Must be able to understand and apply MDS/PPS rules using the Utilization Guidelines for every assessment.
  • Must be able to coordinate the timely development of a written plan of care for each resident including measurable objectives.
  • Must be able to understand the financial concepts and implications of RUG III PPS and state case mix.
  • Must be able to ensure all MDS assessments and RAPs are accurate completed encoded edited printed and exported per state and federal requirements.
  • Must be able to interview residents or their representatives to identify problems relating to care.
  • Must be able to identify and share research recommendations or other information regarding legal liabilities risk management or quality of care.

Position Purpose

Conducts and coordinates the completion of required Resident Assessment Instrument (RAI) and is responsible for the implementation and ongoing evaluation of each residents comprehensive plan of care.

Required Qualifications

  1. A current unencumbered active RN or LPN license is preferred.
  2. Experience with MDS completion preferred.
  3. Resident Assessment Coordinator (RAC) training must be completed within six months of hire. RAC Certification is preferred.

Major Duties and Responsibilities

Coordination of the facilitys Resident Assessment Instrument (RAI) process in accordance with state and federal regulations.

Accurate completion of all MDS assessments and any supporting assessments or clinical documentation.

Implementation and ongoing evaluation of each residents comprehensive plan of care.

Scheduling and leading interdisciplinary care plan team meetings.

Auditing medical records for the presence of supporting documentation for all items coded on the MDS. Provide education to department heads physicians and other staff as needed.

Collaborating with health plans physicians and the interdisciplinary team for assuring insurance approvals and/or that residents meet eligibility and coverage criteria.

Interpreting rules regulations and coverage guidelines and acting as primary resource for problem solving in regards to the SNF prospective payment system and quality reporting program.

RAI Coordinator Assigned Tasks

  1. Utilizes the current RAI Manual as a resource during the assessment coding process.
  2. Schedules the Assessment Reference Dates (ARDs) for payment assessments in a manner that accurately captures each residents clinical characteristics for payment classification purposes.
  3. Schedules the ARDs for all OBRA (Omnibus Budget Reconciliation Act) assessments daily monthly and as needed.
  4. Provides a schedule of ARDs and assessment types weekly and as needed to the interdisciplinary team (IDT) in order to facilitate the timely completion of MDS sections and CAAs (Care Area Assessments) by each discipline.
  5. Communicates with members of the IDT as needed for timely completion of assessments. Expected to report any issues with timeliness to the Administrator.
  6. Transmits assessments in accordance with current regulations. To facilitate timely receipt of validation reports will transmit as frequently(daily) as necessary to obtain timely validation of MDS acceptance into the Quality Improvement and Evaluation System (QIES) Assessment Submission and Processing (ASAP) System data base.
  7. Confirms transmission files by review and printing of initial and final validation reports.
  8. Corrects any fatal errors immediately and retransmits the assessment until an accepted validation report is received.
  9. Addresses non-fatal errors using the QIES ASAP System MDS 3.0 Provider Users Guide.
  10. Ensures the Business Office designee receives the initial and final validation report that acknowledges the acceptance of the MDS into the QIES ASAP.
  11. Facilitates the completion and updating of resident care plans with the IDT to reflect each residents current needs.
  12. Communicates with the IDT to identify residents in need of a Significant Change in Status assessment based on criteria in the current RAI manual.
  13. Participates in all resource utilization and/or triple check meetings prior to billing of claims associated with payment assessments.
  14. Assists with admission discharge or care of residents as needed.
  15. Attends or participates in continuing education related to the RAI process and related programs. Completes all assigned training and skills competency as determined by the facility assessment and facility training plans.
  16. Performs administrative duties as assigned including but not limited to QAPI activities and participation in department or committee meetings.

  3. Follows appropriate safety and hygiene measures at all times to protect residents and themselves.
  4. Maintains confidentiality of protected health information including verbal written and electronic communications.
  5. Reports noncompliance with policies procedures regulations or breaches in confidentiality to appropriate personnel. Reports any retaliation or discrimination to HR or compliance officer.
  6. Reports any allegations of abuse neglect misappropriation of property exploitation or mistreatment of residents to supervisor and/or administrator. Protects residents from abuse and cooperates with all investigations.
  7. Reports any occupational exposures to blood body fluids infectious materials and/or hazardous chemicals in accordance with facility policy.
  8. Participates in all life safety and emergency drills and trainings. Fulfills responsibilities as assigned during implementation or activation of the facilitys emergency plan.
  9. Reports work-related injuries and illnesses immediately to supervisor.
  10. Follows established infection control policies and procedures.
  11. As a condition of employment completes all assigned training and skills competency.

  12. Ability to bend stoop kneel crouch perform overhead lifting and perform other common physical movements as needed for the position.
  13. May be subject to falls burns from equipment and/or odors throughout the day; encounter reactions from dust tobacco smoke disinfectants and other air contaminants.
  14. Subject to exposure to infectious waste diseases and/or conditions which include AIDS Coronavirus Hepatitis B and Tuberculosis.
  15. May be subject to hostile or emotional residents family members visitors or personnel.
  16. Regular attendance is a requirement of this position.
  17. Effective verbal and written communication skills and ability to exercise judgement.
  18. Intermediate computer skills including ability to create documents in various programs (Word Excel PowerPoint) and navigate electronic medical record systems.
  19. Ability to work beyond normal working hours and on weekends and holidays when necessary.
  20. Ability to work independently and to organize plan and manage time effectively to complete assignments.
  21. Positive interpersonal relationship skills including with persons of all ages and cultures.
  22. Current awareness of relevant regulations and standards of care.
