What Youll Do as a Physical Therapist
Youll help patients maximize their independence mobility and quality of life right in their own homes. Responsibilities include:
- Delivering direct physical therapy services with excellence
- Evaluating functional needs and updating care plans accordingly
- Designing customized outcome-based therapy plans
- Collaborating with family members and care teams
- Guiding and supporting therapy assistants
- Maintaining accurate documentation goals and progress notes
- Acting as a community and clinical resource
- Working in a physically active fast-paced care environment
- Safely lifting transferring and assisting patients as needed
What Youll Need
- Graduate of an APTA-accredited PT program
- Unrestricted Indiana PT license
- 1 year of healthcare experience
- Reliable vehicle drivers license and current auto insurance
- Ability to travel within the Mishawaka service region (up to 50%)
- Ability to lift 50100 lbs and meet physical demands
Reporting To:
Branch Director Clinical Manager Clinical Supervisor or designee.
