Make it Happen at Hopkins NOW HIRING Full Time Care Manager Hybrid

Join our team and be part of a newly established care management program dedicated to serving our community by delivering high-quality patient-centered care. The RN Care Manager is an integral member of the primary care team working collaboratively with providers to engage patients in improving chronic disease self-management coordinating care and reducing unnecessary healthcare utilization.

In this role you will support high-risk patients with complex medical and social needs by coordinating care across multiple providers and settings providing chronic disease education performing medication reconciliation and education and helping patients navigate the healthcare system. You will play a key role in implementing programs designed to improve health outcomes and reduce avoidable hospital and emergency department use.

Hybrid position: 4 days in office 1 day remote

Schedule: Monday - Friday 8:00am - 5:00pm

Education Licensure and Certification:

  • Bachelors Degree from accredited school of nursing.
  • Current MD RN License or multi-state or compact required

Experience and Qualifications:

  • 5 years minimum as RN 3 years of care managment.
  • Managed Care experience preferred
  • Community-based actue care setting experience preferred
  • Experience with EPIC Compass Rose highly preferred

Salary Range: Minimum 42.34/hour - Maximum 65.62/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount salary discussions will occur during candidate screening calls before any subsequent compensation discussion is held between the candidate and any hiring authority.
