What You Will Do

Manage and coordinate your own patient caseload
Complete initial and ongoing patient assessments
Develop implement and update individualized care plans
Perform start-of-care visits and routine follow-up visits
Communicate patient condition changes with physicians and care team
Maintain accurate documentation and HIPAA compliance
Support patients and families throughout their care journey

What You Need

Current unrestricted Registered Nurse (RN) license
Minimum 1 year of clinical nursing experience
Ability to travel within the branch coverage area
Reliable vehicle valid drivers license and current auto insurance
Ability to frequently lift push pull or support up to 50 pounds
Associate or Bachelors degree in Nursing preferred
Home Health or Hospice experience preferred
Experience with HomeCare HomeBase preferred
