Schedule: Monday Friday 8-Hour Days 40 Hours Per Week
Work hours are flexible provided they remain consistent and are coordinated with the Director of Rehabilitation (DOR).
Position Overview
Trinity Healthcare Talent Services is seeking a Travel Physical Therapist (PT) for a long-term care rehabilitation setting in Buffalo MO.
The Physical Therapist will evaluate and treat patients experiencing physical mobility limitations chronic conditions or rehabilitation needs. The therapist will develop individualized treatment plans to restore function reduce pain and improve independence in daily activities.
This role requires collaboration with healthcare teams and adherence to clinical practice standards to ensure effective patient care.
Key Responsibilities
Conduct patient screenings evaluations and functional mobility assessments
Develop individualized rehabilitation treatment plans
Implement therapeutic exercises manual therapy techniques and rehabilitation modalities
Provide gait training and functional mobility assistance
Utilize assistive devices and supportive equipment when necessary
Monitor patient progress and adjust treatment plans accordingly
Document clinical interventions and patient progress in the electronic medical record system
Provide patient and caregiver education regarding treatment plans and rehabilitation goals
Participate in care conferences utilization reviews and interdisciplinary team meetings
Supervise Physical Therapist Assistants (PTAs) rehabilitation aides or students as needed
Patient Population
Patients may include individuals with:
Mobility impairments due to injury or illness
Chronic musculoskeletal conditions
Post-operative rehabilitation needs
Functional limitations requiring long-term rehabilitation support
Required Qualifications
Minimum 1 year of Physical Therapist experience
Long-Term Care (LTC) or rehabilitation experience required
Active Missouri Physical Therapist License
New Graduates
New graduates may be considered if they have completed Level II clinical experience
Scheduling & Assignment Details
40 hours per week
Monday Friday schedule
No on-call duties
No weekend requirements
