Job Summary:

We are hiring an LPN/LVN for a Long-Term Care (LTC)/SNF setting. The candidate will provide quality nursing care assist with patient monitoring and support daily clinical operations.

Key Responsibilities:

Provide direct patient care in LTC/SNF settings.

Monitor patient conditions and report changes

Administer medications and treatments

Maintain accurate patient documentation (PointClickCare eMAR)

Support care for patients with dementia chronic diseases and diabetes

Requirements:

Active NY LPN License (Required)

BLS Certification (AHA) required

1 3 years of LPN experience

LTC or SNF experience required
