Job Summary
Under the direction of the IHA Service Lead the Operational Team Lead Integrated Care Programme for Older Persons will be responsible for implementing the Service Model for Older Persons for the IHA. While not directly involved in delivery of care they will have experience in the delivery of older persons services to manage the ICPOP/ ICPOP related Community Specialist Team.
The ICPOP lead will manage a patients care journey and signpost patients to the correct service coordinate patients treatment plans connect them with health care professionals and evaluate their progress.
The role is multifaceted and includes:
1. Responsibility for the day-to-day operational management of the Integrated Care Team (Older People) and the hub functions.
2. Develop and manage Operational Policies that captures the functions of the hub/s and the MDT roles and functions
3. Operationally manage staff in the Hub
