The Discharge Planning Coordinator is responsible for designing and coordinating comprehensive discharge plans in accordance with regulatory and patient satisfaction standards. Collaborates with nurse case managers and social workers regarding overall discharge planning needs.Education
Bachelors Degree Accredited School of Social Work Required
Experience
1 year experience with referral coordination Preferred
2 years experience in a Hospital Clinical setting Preferred
