Job Summary
Empath Partners in Care a part ofEmpath Healthis seeking a compassionateLead Medical Case Managerto join our team inSt. Petersburg FL. In this role the Lead Medical Case Manager will provide direct support and guidance to medical case management staff. This position will collaborate with the Case Management Coordinator to ensure all employee issues are properly addressed. This position will provide case management services to the HIV/AIDS population as they request such services.
What Youll Do
- Participate in ongoing HIV/AIDS training and remain current on medical and medication updates; educate clients on transmission prevention and available resources.
- Maintain annual certification as a Certified Application Counselor (ACA) and assist clients with enrollment.
- Provide daily consultation guidance and resource support to medical case managers.
- Maintain client contact and documentation in accordance with DOH and PAC Waiver guidelines.
- Meet required performance standards including billable unit expectations documentation accuracy and compliance metrics.
- Complete new client intakes and ensure timely follow-up and referral completion.
- Train new employees using established case management manuals and protocols.
- Review case manager documentation for accuracy and clarity providing feedback and required corrections.
- Collaborate with leadership on corrective action steps and review client file closure requests.

What Youll Need
- Bachelors degree (B.A.) from four-year college or university; or one to two years related experience and/or training; or equivalent combination of education and experience.
- Case Management experience or at least one year work experience in advocacy services or similar public work in direct services or educational background in the social services field.
- Flexibility to attend meetings and trainings which may fall outside the regularly assigned work hours.
- Excellent communication skills and ability for detail.
- Sensitivity to cultural differences and alternative sexual orientations to include the Gay Lesbian Bi-Sexual and Transgendered Communities.
- Sensitivity to the psychosocial issues concerning individuals living with HIV/AIDS.
- Must have reliable transportation which will enable them to perform tasks and responsibilities in a timely and appropriate fashion. Must provide proof of valid automobile insurance
