Job Description:
This opportunity is based in Montrose CO serving patients through HopeWest Hospice. This role offers the chance to practice truly patient-centered care while making a meaningful difference in the lives of patients and their families.
As a Hospice RN you will provide compassionate holistic care and serve as both a clinician and educator-guiding patients and caregivers through care plans symptom management and end-of-life support.
Primary RNs manage a dedicated caseload and lead the interdisciplinary team ensuring personalized care wherever the patient calls home including:
- Private residences
- Nursing homes
- Assisted living facilities
- Group homes
Shift Information:
- Monday to Friday (8:00 AM 5:00 PM)
- Occasional weekend backup and on-call responsibilities
Responsibilities:
- Manage assigned patient caseload
- Lead and coordinate interdisciplinary care teams
- Provide symptom management and clinical assessments
- Educate patients and caregivers on care plans and goals
- Deliver care across multiple home-based settings
Requirements:
- Active RN license
- Strong clinical and assessment skills
- Experience in hospice or home health preferred
- Ability to work independently and manage caseload effectively
