Outreach and Sustainment Officer

We are working closely alongside with a local authority in Waltham Forest to assist with the appointment of a Outreach and Sustainment Officer on a 3-months contract highly likely to be extended at clients discretion. Please apply with your CV for immediate consideration.

Rate of Pay: 17.53 - 23.12 per hour

Summary:

The Outreach and Sustainment Officer is a dual-purpose role focused on both crisis intervention and long-term stability. You will deliver proactive trauma-informed outreach to identify and engage individuals sleeping rough or at risk of doing so ensuring they receive urgent safety checks and access to accommodation. Beyond the streets you will provide critical sustainment support from the moment an individual moves into temporary or independent housing helping them stabilize build independence and successfully maintain their tenancy to prevent a return to homelessness.

Responsibilities:

• Conduct proactive outreach to identify and engage people sleeping rough or those at immediate risk of rough sleeping.
• Perform regular safety checks and provide timely intervention to ensure the immediate well-being of vulnerable individuals.
• Assess individual needs risks and personal circumstances to create tailored support pathways.
• Provide clear expert advice on housing health welfare benefits and specialist services to help individuals move away from street life.
• Assist individuals in settling into their new environment helping them build the skills necessary for independent living.

Essentials:

• Deep understanding of trauma-informed care and its application in a housing or outreach context.
• Strong working knowledge of homelessness legislation and the welfare benefits system.
• Proven experience in identifying and engaging hard-to-reach individuals with complex needs (e.g. substance misuse mental health or dual diagnosis).
• Demonstrable experience in assessing needs and developing successful move-on or sustainment plans.
• A full UK driving license
