Responsibilities:
Develop and roll out provider education programs regarding sexual health with the goal of increasing the number of local primary care providers who can provide comprehensive testing and prevention services. Activities may include development of provider education materials designing template sexual health workflows for provider offices and in-person visits to provider offices to share information and provide technical support regarding implementing new policies and practices.

Provide health-related information and resources as needed and as requested by community groups and other stakeholders to include offering invited talks and written educational materials as appropriate to community needs.

Evaluate the impact of provider education interventions using surveys service data (if available) and public health data as appropriate.

Provide high-quality culturally competent primary care to patients including assessments of health status diagnosis and treatment of common health problems and management of overall health and chronic illnesses.

Collect record and maintain patient information completing patient charts and billing entries in a timely manner.

Provide patient education as appropriate including advice regarding diet activity hygiene and preventive measures to avoid illness.
Refer patients to specialists or other practitioners when appropriate; consistently document and follow up on referrals as needed.
Serve as a strategic liaison between the health system and community-based organizations public health agencies advocacy groups and external stakeholders to expand access to sexual health prevention services.
Lead community needs assessments and utilize epidemiologic data to inform outreach strategy program development and targeted prevention initiatives.
Develop implement and monitor grant-funded or externally supported HIV/STI prevention initiatives in collaboration with public health partners.
Represent the organization at community advisory boards public health coalitions and regional prevention collaboratives.
Design and oversee mobile or off-site clinical outreach initiatives including community-based testing events and prevention campaigns.
Establish and track population health metrics related to PrEP uptake Doxy-PEP utilization and STI screening rates.
Provide technical assistance to external provider practices on implementation of sexual health best practices and prevention protocols.
Collaborate with Marketing and Community Affairs to develop culturally responsive outreach materials and public health messaging campaigns.
Participate in policy development and strategic planning related to LGBTQ health equity initiatives across ambulatory and community settings.
Maintain flexibility to provide services in non-hospital environments including community centers outreach sites and partner agencies.

Other Duties:
Performs other duties as assigned to support the overall objectives of the department and organization.

Position Qualifications:
Current NJ licensure and certification as a Nurse Practitioner and RN
BLS ACLS and PALS certification required
DEA certificate
Experience with or demonstrated knowledge of sexual healthcare including testing prevention and treatment.
Experience with or demonstrated knowledge of primary care for the LGBTQ community.

Education:
Graduation from an approved Nurse Practitioner program is required.
