Job Summary

Job DescriptionThe Transition Job Coach supports students ages 1822 with disabilities in community-based and school-based vocational placements. The Job Coach provides direct on-site coaching skill training and fading supports to help students build work readiness job competence and independence aligned with IEP transition ExperienceExperience working with individuals with disabilities preferably transition-age studentsExperience in vocational training job coaching or community-based instructionExperience implementing behavior and support plansExperience collecting performance data and documenting progressExperience supporting independence and adaptive skill developmentCore ResponsibilitiesProvide on-site job coaching at student work placementsTeach and reinforce job tasks using task analysis and systematic instructionSupport development of:Work habits and staminaAppropriate workplace behaviorCommunication with supervisors and coworkersProblem-solving and self-advocacyImplement IEP transition and employment goalsUse prompting and planned fading strategies to increase independenceCollect job performance data and complete coaching logsCommunicate regularly with the Transition Special Educator and program staffCoordinate with employers and site supervisors under staff directionSupport travel training and community access as assigned
