Job Summary
The Registered Nurse (RN) Case Manager is responsible for coordinating patient care ensuring appropriate utilization of healthcare services and supporting clinical documentation improvement initiatives. This role involves collaborating with physicians nurses and interdisciplinary teams to manage patient care plans monitor resource utilization and facilitate safe and efficient patient care delivery.
The RN Case Manager works within an acute care hospital environment and plays a key role in reviewing patient cases supporting discharge planning and ensuring compliance with healthcare regulations and documentation standards.
Key Responsibilities
Coordinate and manage patient care plans in collaboration with physicians and multidisciplinary healthcare teams.
Conduct utilization reviews to ensure appropriate use of healthcare resources and services.
Support clinical documentation improvement (CDI) initiatives to ensure accurate patient records and coding.
Review patient cases and medical records to verify compliance with hospital policies and clinical guidelines.
Facilitate discharge planning and care coordination to ensure continuity of care.
Communicate with healthcare providers patients and families regarding care plans and treatment progress.
Maintain accurate and timely documentation in the Electronic Medical Record (EMR) system.
Monitor patient progress and assist with resolving barriers to care or discharge.
Collaborate with internal departments and healthcare teams to optimize patient outcomes.
Ensure compliance with healthcare regulations policies and patient safety standards.
Required Qualifications
Current Registered Nurse (RN) license in New York State.
Basic Life Support (BLS) certification required.
Minimum 1.5 years of recent acute care nursing experience within the past two years.
Pediatric clinical experience required.
Experience working in acute care hospital case management.
Experience using Epic Electronic Medical Records (EMR) system.
Strong communication coordination and problem-solving skills.
Ability to manage multiple cases and collaborate effectively with interdisciplinary teams.
Preferred Qualifications
Experience in Utilization Review (UR) and Clinical Documentation Improvement (CDI) processes.
Experience working in large hospital systems or trauma centers.
Strong knowledge of healthcare compliance and patient care coordination processes.
