**Core Responsibilities:**
- Engage comprehensively in all components of the Case Management Program (CMP) to ensure compliance with established case management (CM) standards of care.
- Leverage nursing expertise throughout the CM process involving assessment planning implementation coordination and monitoring. Identify and capitalize on opportunities to enhance CM efficacy and integrate local CM methodologies.
- Design and execute localized strategies encompassing inpatient outpatient onsite and telephonic case management.- Create and implement tools that underpin case management functions including instruments for patient identification and assessment adherence to clinical practice guidelines algorithms CM software and community resource databases.
- Facilitate the integration of case management with utilization management (UM) and harmonize nursing case management with social work case management.
- Maintain productive relationships with relevant community agencies and organizations.
- Collect and document patient care data meticulously to ensure accuracy and compliance.
- Formulate comprehensive treatment plans that encompass preventive therapeutic rehabilitative psychosocial and clinical interventions to promote continuity of care aimed at achieving optimal wellness.
- Establish robust mechanisms to guarantee effective execution of patient treatment plans and ensure adequate follow-up post-discharge in both ambulatory and community healthcare contexts.
- Deliver tailored health care education to patients and/or caregivers based on assessed learning needs.
**Work Schedule:**- Monday to Friday 8.5-hour shifts between 7:00 AM and 5:30 PM.
- No weekends holidays on-call or call-back duties.
**Minimum Qualifications:**
- **Degree:** At minimum an Associate Degree in Nursing.-
**Education:** Graduates from programs accredited by the Accreditation Commission for Education in Nursing (ACEN) or the Commission on Collegiate Nursing Education (CCNE) are preferred. Alternatively graduates from a State Board of Nursing or other accredited nursing programs may qualify.-
**Certifications:** Candidates must possess one of the following certifications:
- Certified Case Manager (CCM) from the Commission for Case Manager Certification.
- Certified Disability Management Specialist (CDMS) from the Certification of Disability Management Specialists Commission.
- Certified Rehabilitation Registered Nurse (CRRN) from the Association of Rehabilitation Nurses.
- Certified Occupational Health Nurse (COHN) or Certified Occupational Health Nurse-Specialist (COHN-S) from the American Board for Occupational Health Nurses.
- Advanced Certification in Continuity of Care (ACCC) from the National Board for Certification in Continuity of Care.
- Certified Rehabilitation Counselor (CRC) from the Commission on Rehabilitation Counselor Certification.
- Nurse Case Manager (RN-NCM) from the American Nurses Credentialing Center.
- Care Manager Certified (CMC) from the National Academy of Certified Care Managers.-
**Experience:** Minimum of one year of post-graduation nursing experience.-
**Licensure:** A current full active and unrestricted Registered Nurse license from any state.-
