PACE a member of Empath Healthis seeking a compassionate Community Outreach Specialistto join our interdisciplinary PACE team in Bradenton FL. In this role the PACE Community Outreach Specialist is responsible for driving program growth through strategic community engagement relationship development and high-quality referral generation.

The Program of All-Inclusive Care for the Elderly (PACE) is a unique care model designed to help older adults remain safe and independent in their own homes.
- Relationship-Based Care with Real Impact: Build long-term connections with participants supporting them through all stages of carethis is a meaningful alternative to traditional inpatient or facility-based care.
- Team-Driven Preventive Focus: Collaborate closely with physicians social workers therapists and other clinicians in a proactive care model focused on prevention wellness and holistic well-being.
What Youll Do
- Build and maintain strong relationships with community partners referral sources senior-serving organizations and healthcare providers.
- Develop and execute outreach strategies to increase awareness of the PACE program.
- Generate high-quality referrals and guide prospective participants and families through the enrollment process..
- Represent the PACE program with professionalism empathy and a deep understanding of geriatric care needs.
- Meet and exceed performance metrics tied to outreach activity referral volume and enrollment growth.
- Identifies populations of Medicaid Medicare Dual Eligible VA and Private Pay individuals and strategically aligns Empath Life PACE services within their communities through referral channels.
- Serve as a brand ambassador in the community strengthening program visibility and reputation.
- Maintain a thorough knowledge of the communitys health care market competition consumer demands and governmental policies which influence the agencys outreach goals and access for patients/families.
- Creates and conducts effective proposal presentations to referral sources and prospective participants.

What Youll Need
- Bachelors Degree in marketing or healthcare administration preferred
- Minimum of two (2) years of experience in health plans or senior-focused programs with demonstrated success meeting or exceeding sales goals
- At least two (2) years in a healthcare or senior care setting preferred (experience within the service area and outside sales experience strongly preferred)
- Strong customer service orientation
- Ability to work independently with minimal supervision
- Experience with CRM software preferred
- Bilingual skills a plus
